• County "E" Level ID Information Form
  Use this form to request a new E level County Administrator.
• Reschedule Report Record Removal Request
  Use this form when County requests that clients no longer on aid be removed from Report CS4219MW Photo Retake Over 60 Days Old or CS4305M Fingerprint Exemption Report.
• SFIS Brochure Request
  Use this form to request more SFIS Brochures for your office.
• SFIS Training Enrollment Form
  Use this form to request enrollment into our SFIS Classroom Training.
• SFIS Web Based Training (WBT) Evaluation Form
  Use this form to evaluate the SFIS Web Based Training Modules.
• SFIS Workstation Security Form
  Use this form when SFIS equipment (workstation, portable, zip disk) is lost or stolen.
• County Coordinator Information
  Submit this form to update information and/or to change the SFIS county coordinator. This form MUST be filled out by the previous and/or current County Coordinator listed on our SFIS County Coordinator List.
• Authorized to Activate SFIS Direct Training Form
  Submit this form to update information and/or to change personnel authorized to request the activation of SFIS DT on a production workstation. This form MUST be filled out by the County Coordinator listed on our SFIS County Coordinator List.
• Authorized to Request Image Removal Form
  Submit this form to update information and/or to change personnel authorized to request image removal. This form MUST be filled out by the County Coordinator listed on our SFIS County Coordinator List.
